Win/Loss Request

In order for Boomtown Reno to release information regarding your tracked play you must complete and submit a signed Win/Loss Request Form.

Once the Request Form is completed you can submit the form for processing in one of three ways:

1. By mail to BO Box 399, Verdi, NV 89439
2. By fax to (775) 345-8776
3. In-person

Please allow 2-4 weeks for request processing. 2009 tax year requests will be processed in date order received.
